Getting Started

Most claims are processed and paid within 2 weeks. Providers can choose between receiving payment in the form of a check or electronically. (To learn more about electronic payment, click here.)

Submit a Claim

FamilyCare Health recommends electronic claim submissions, which eliminate errors and speed up the claims process. Our electronic claims are processed through the Community Integration Manager (CIM) tool, which is managed by PH Tech, our third-party administrator.

If you are a contracted provider with FamilyCare Health and need CIM access, please call us at 503-222-2880 or toll-free 800-458-9518. You can also use this form to request access. If you have difficulty using CIM, please call PH Tech CIM Technical Support at 503-584-2169 or email edi.support@phtech.com.

Your team can submit claims to either of our two EDI submitters: Emdeon or OptumInsight.  

•    For Emdeon EDI Submitters:

     Group ID: 218751 (Medicaid)

•    For OptumInsight EDI Submitters:

     Payer ID: PHR01 (Medicare)

We do still accept paper claims using forms UB04 or CMS 1500. (We do not supply these forms.) Completed forms can be mailed to:
