Medication Therapy Management

2017 Medication Therapy Management

If you have several complex health conditions or take many drugs, our no-cost Medication Therapy Management (MTM) program is designed to help you manage your drugs and cut your risk for problems. A pharmacist will work with you and your doctor to help make sure the drugs you are taking are safe and work well for you. We will look at all of your drugs, including prescription and over-the-counter drugs, vitamins, and herbal products. All members, including those not on MTM, can use this personal medication list to keep track of their medications. 

You will be automatically enrolled in MTM if you:

  • Take eight or more Medicare Part D-covered maintenance drugs, and
  • Have three or more long-term health conditions, such as diabetes, asthma, or high blood pressure, and
  • Spend more than $3,919 a year on your drugs.

If you qualify, we will notify you by mail, phone, or email and invite you to have a comprehensive medication review (CMR) with a pharmacist. The pharmacist will take about 15-30 minutes of your time to help you better understand your medications and make sure your medications are the safest for you. After the review, you will get a personal medication plan and medication list to help you with managing your medications. The CMR is usually performed over the phone but can also be performed at your local pharmacy. Ask if your local pharmacy provides this service on behalf of FamilyCare Health. 

Please note that not all plan members will qualify for the MTM program, therefore the program is not considered a benefit.

You can reach us from 8 a.m. to 8 p.m., seven days a week, from October 1 through February 14 (except Thanksgiving and Christmas*). From February 15 to September 30, call us Monday through Friday from 8 a.m. to 8 p.m. and on weekends from 9 a.m. to 5 p.m. (except Memorial Day, Independence Day, and Labor Day*).
