2017 FamilyCare Community (HMO SNP)
For those who qualify for both Medicare and the Oregon Health Plan (Medicaid), this $0 premium plan combines your coverage and includes up to a $200 yearly credit towards eyeglass frames when using VSP services, a $60 monthly credit for over-the-counter health aids, and up to a $40 a month fitness club reimbursement.
