Being overweight can be hazardous to your health. If you have a Body Mass Index (BMI) of 25 or higher, you're more likely to develop high blood pressure, type 2 diabetes, heart disease, stroke, and even some cancers. Find out your BMI with this calculator.

If you'd like to lose weight, FamilyCare Health is here to help.

FamilyCare Health has partnered with a number of providers to help our members find and maintain a healthy weight, even if you're busy or living on a tight budget. Here are two ways we can support you on your journey to better health:

1. We'll help you find a PCP that specializes in weight loss.

If you want individualized care, including help with what to eat and how much to exercise, you might want to find a primary care provider that specializes in weight management. Some providers we contract with focus on lifestyle changes and weight loss as preventive care. For example, family nurse practitioner Seth Merritt, of Merritt Health and Wellness, specializes in educating and supporting patients throughout the weight loss process. If your primary care provider is at SW Family Physicians, they can also help with weight loss.

Medicare members can call 503-345-5702.

2. We'll help you sign up for a weight loss program.

If you prefer following a weight loss plan in a group setting, you might benefit from one of the weight loss programs FamilyCare Health covers. One of our more popular programs is the 10- to 12-week Complete Health Improvement Program (CHIP), which is offered by the Lifestyle Medicine Institute. This program, which is free to FamilyCare members, includes classes, wellness screenings, and materials you can take home, such as cookbooks and workbooks. With the support of the instructors and the other people in the program you'll learn how to better take care of your health.

To find out if you're eligible for this program, contact your PCP.
