Patient Centered Primary Care Homes

Across Oregon, health care clinics are finding new and better ways to care for you. Patient-Centered Primary Care Homes are not really homes, but places where you are taken care of, just like home. These places are clinics that have been recognized for their commitment to a patient-centered approach to care. Just as it sounds, patient-centered care is all about you—the patient.Patient-centered care is a team-based approach to care focused on prevention and managing health conditions. By coordinating the needs of patients through a team of health professionals, more focus can be on prevention and delivering needed care. Care is comprehensive and coordinated across all aspects of care, including physical, mental, and specialty care, which is especially important for patients who receive multiple types of care from a variety of health care providers.

At Patient-Centered Primary Care Homes, you are the most important part of your care. Your personal health care team will:

  • Coordinate your care, getting you the services you need, when you need them
  • Listen to your concerns and answer your questions
  • Offer after-hours help and alternatives to the emergency room
  • Help you play an active role in your health

Your primary care provider will work with you to improve care coordination, which results in higher quality care to you and your family. If needed, you will be connected with other health professionals to help you receive the care you need. Patient-Centered Primary Care Homes provide:

  • An interdisciplinary team that guides care in a continuous, accessible, comprehensive, and coordinated manner
  • Responsibility for coordinating its patients’ health care across care settings and services over time, in consultation and collaboration with the patient and family
  • Ready access to physical and mental health care
  • Care that is whole-person oriented and consistent with patients’ unique needs and preferences from a team who “knows” you
  • Patient and clinician partnerships in making treatment decisions
  • Care where open communication is encouraged and supported between the patient and their care team

As a leader in innovative systems, FamilyCare Health Plans offers affordable, high quality, patient-centered health plans to Oregonians receiving benefits through Medicare or the Oregon Health Plan. Recognized clinics can now apply to receive additional Medicaid funding to support the comprehensive, coordinated, and patient-centered care they offer Medicaid patients with chronic conditions such as diabetes and asthma.
