At Patient-Centered Primary Care Homes, you are the most important part of your care. Your personal health care team will:

• Responsibility for coordinating its patients’ health care across care settings and services over time, in consultation and collaboration with the patient and family

Recognized clinics can now apply to receive additional Medicaid funding to support the comprehensive, coordinated, and patient-centered care they offer Medicaid patients with chronic conditions such as diabetes and asthma.
