We are pleased to provide the FamilyCare Medicare Formulary as a useful reference and informational tool. The FamilyCare Medicare Formulary can assist practitioners in selecting clinically appropriate and cost-effective drugs and products for their patients.

The drugs represented have been reviewed and approved by FamilyCare’s Pharmacy and Therapeutics (P&T) Committee. The FamilyCare Medicare Formulary is reflective of current medical practice and the Oregon Health Authority’s coverage guidelines as of the date of review.

All the information in the FamilyCare Medicare Formulary is provided as a reference for drug therapy selection. Specific drug selection for an individual patient rests solely with the prescriber.
