How to Enroll

Many Oregonians on the Oregon Health Plan (Medicaid) depend on FamilyCare for their healthcare needs. We serve members from a wide variety of backgrounds, with a range of needs to ensure their health care needs are adequately and efficiently met. We assign Care Coordinators to work one-on-one with members in identifying potential barriers and connecting them with personal health care services and primary care providers. We fully-integrate mental and physical health care services to ensure comprehensive patient care to our members.
