About FamilyCare Health Plan
We are proud to be an Oregon-based health plan, founded specifically to serve people on the Oregon Health Plan (Medicaid). For the last 30 years, our innovative approach to healthcare has created solutions for members to take charge of all aspects of their health. We were the first Medicaid health plan in Oregon to integrate mental and physical health coverage into one plan (we now offer dental care services). We became the first health plan in the tri-county (Clackamas, Multnomah and Washington) area certified as a coordinated care organization (CCO) under the Oregon Health Authority. Becoming a member of FamilyCare
First, you need to know if you are eligible for the Oregon Health Plan. Beginning in January 2014, the Oregon Health Plan (OHP) is opening to more adults and you may now qualify. There are several ways to check:
- Online: You can apply through Cover Oregon, www.coveroregon.com. Cover Oregon is the new online marketplace where Oregonians can shop and compare health insurance. You can find out if you qualify for OHP or other types of coverage. If you earn too much for OHP, there may be financial assistance available for other health plans.
- Community Partner: You can visit the Cover Oregon website for a list of community partners in the Clackamas, Multnomah and Washington counties. These community partners reflect the diversity of our communities and are trained to help you apply and enroll. For a list of helpful groups, go to www.coveroregon.com/agent/find
- Phone: You can call 855-CoverOR. For questions about FamilyCare you can call us at 503-222-2880 or toll-free at 800-458-9518 (for TTY, call 711), Monday through Friday, 9 a.m. – 5 p.m.
- In person: You can find an application at any Oregon Department of Human Services office. Go to www.ohp.oregon.gov for a list of locations or call 855-CoverOr. You can request a paper application in the language needed.
Fast-track enrollment for the Oregon Health Plan
You may have received a fast-track letter from the state of Oregon. Fast-track enrollment is available to adults who are qualified to receive food benefits through the Supplemental Nutrition Assistance Program (SNAP) or either have children enrolled on the Healthy Kids/Oregon Health Plan. If you did receive a letter, simply fill out fast-track enrollment form enclosed with the letter and return it. If you did not receive a letter, call the Oregon Health Plan at 800-699-9075 to speak with a customer service representative or go to www.oregon.gov/oha/healthplan/Pages/fast-track.aspx.
