Application Process

Complete and sign: Enrollment Form, Quick Pay for (if needed), Documentation of Authorized Representative (POA) if applicable
Provide a copy of: Medicare ID card, LIS Letter if applicable, DMAP ID card if applicable, Letter of Credible coverage if applicable
Once FamilyCare receives and verifies the enrollment you will receive a confirmation from Medicare.
